Services to Coordinate and Deliver Trials under the Aged Care System Navigator Measure

Description
The trials will test a range of different system navigator models in different circumstances that support people who need additional support to understand and engage with the aged care system to:
- Understand the aged care system, including what services are available to meet their needs and how to access them.
- Engage with and access the aged care system, including connecting these people with My Aged Care and providing them with support to choose and access services.
The trials include:
- 30 aged care information hubs to provide locally targeted information and build people’s capacity to engage with the aged care system.
- 20 community hubs where members support each other in navigating aged care and healthy ageing.
- Six full-time equivalent specialists in consumer-focused organisations to offer one-on-one support for vulnerable people.
The trials will be spread across a range of locations, including a spread across states and territories, metropolitan and non-metropolitan areas, and different demographic/population groups. The target population is people who:
- Have difficulty engaging through the existing channels and need additional support to understand, choose and access aged care services (with a focus on people who face barriers to accessing aged care services and people who are vulnerable).
- Have not yet accessed aged care services.
The trials are expected to commence in January 2019 and conclude in June 2020. Their outcomes will be used to inform future decision making.
The successful Tenderer will:
- Coordinate implementation and deliver the trials in a manner that achieves their aims and objectives.
- Coordinate establishment and management of required data and information systems across the trials to support the evaluation of the Aged Care System Navigator measure.
- Coordinate collection and reporting of data and information across the trials to support the evaluation of the Aged Care System Navigator measure.
The successful Tenderer will be responsible for all aspects of implementation and delivery of the trials. A detailed statement of requirement is set out in the Request For Tender document.
