Description
The Western Australian Department of Health requires the provision of a state-wide, technology-based, health coaching service (delivered by tertiary qualified health professionals) for individuals, targeting chronic condition modifiable risk factors through early intervention to empower lasting behaviour change.
This Community Services Request for Community-Based Chronic Conditions Early Intervention Program replaces the previous Request withdrawn from Tenders WA on 8 February 2023. Organisations are invited to submit an offer in response to this Request by 3pm Wednesday 7 June 2023
