Description
The Department for Health and Wellbeing is seeking to engage a suitable supplier to deliver the My Home Hospital (MyHH) services for a period of up to nine (9) years, commencing on 1 October 2025. MyHH is a critical health initiative aimed at supporting patients within their home environment and is an important part of the SA Health healthcare landscape. MyHH has been developed to provide a holistic, end-to-end, home-based acute care service delivery model to support Local Health Networks (LHNs) to meet current and future demand. SA Health patients who reside within metropolitan or peri-urban Adelaide can access MyHH services that are patient centred and safe, evidence based and medically led in a home setting, including private homes and residential aged care homes/disability support accommodation. |
