Panel Contract for Individually Funded Services

The Disability Service Commission (State Party) is seeking responses to expand the Panel Contract of Service Providers to deliver a range of diverse, innovative and flexible services to people with disability, living in the Perth metropolitan region, the Western Australian (WA) National Disability Insurance Scheme (NDIS) National Disability Insurance Agency (NDIA) trial site in the Perth Hills region, the WA NDIS My Way trial sites in the Lower South West, and post 1 July 2015 the WA NDIS My Way trial Cockburn-Kwinana regions of Western Australia. The delivery of services will be in accordance with individual plans and funding packages.

Respondents can provide a response for either or both parts.

Service Providers who have service agreements with the Mental Health Commission must apply and be successful in this request process to provide services in the WA NDIS My Way and WA NDIS NDIA trial sites and/or expand their services to include disability services.

Service Providers who are already on the Panel Contract for Individually Funded Services do not need to reapply during this process.

Under NDIS Guidelines, registration to become a NDIS Service Provider must have been through the Service Provider approval process of the relevant state jurisdiction as well as registering with NDIA. Therefore, to be a Service Provider in the WA NDIS My Way and NDIA trial sites in Western Australia a Service Provider must be on the State Party’s Panel Contract for Individually Funded Services
