The SNF program is a sustained health home visiting program where specially trained child and family health nurses visit families with identified risks and vulnerabilities, commencing during pregnancy until the child is 2 years old. The nurses are supported by a funded multidisciplinary team. Staff receive specialised program training through the SNF Training Course.
