Description

The Coordinated Veterans’ Care (CVC) Program is a planned and coordinated health care model for eligible DVA Health Card (Gold) holders with one or more chronic conditions, complex care needs and who are at risk of unplanned hospitalisation.

The model of health care within the CVC Program involves an ongoing partnership between a core team and is based around the development of a personalised comprehensive Care Plan. The core team consists of the veteran, the veteran’s carer (if applicable), the General Practitioner and a nurse coordinator. The team uses care planning, coordination and review to focus on planned and coordinated management and self-management support of the veteran’s health.

DVA is seeking the services of a suitably qualified Training and Resources Provider to support the effective delivery of the CVC Program to December 2019. The successful tenderer will have expertise in adult health education and health customer service and capacity to provide training primarily in an online format as well as face to face where required. Capacity to host and maintain a training and resources website is also a requirement.
