Description
Purchase Order Disclosure – Continue to support existing Telehealth projects at Nepean Blue Mountains Local Health District by providing research, advice and recommendations for project upgrades and developments and maintenance of existing systems (including Apps). Establish a pilot to facilitate early discharge and improved management of patients with chronic disease through Telehealth, and identify and engage stakeholders regarding opportunities for new Telehealth projects.
