Description

The Queensland Government is seeking Proposals for co-design and evaluation of the Queensland social prescribing trial. A collaborative process of local engagement, consensus-building and decision-making about model design and an evaluation approach, and ongoing monitoring and evaluation, fulfilling the following as a minimum:

  • Facilitation of face-to-face co-design workshops and/or activities in the 10 locations to support stakeholder onboarding to the trial and to develop the place-specific components of the model for service delivery within each of the trial locations, and the approach to learning and evaluation at a place-level.
  • Design should support scoping of the place-specific functions of the Social Prescribing Link Worker and the social prescribing referral mechanisms. This includes (but is not limited to) practice frameworks, relevant assessment and referral tools, and an approach to mapping of local assets, networks and infrastructure.
  • Co-design of an evaluation framework for the whole trial, outlining evaluation principles and an adaptive approach, with a strong focus on learning.
  • Provision an interim and a final report, including evidence of any ROI and SROI.
  • Strong partnership formation and ongoing close engagement with relevant stakeholders in the trial (particularly the Social Prescribing Link Workers and the departmental project lead) within a clear governance structure, with documented decision-making processes, determined in partnership with the department.
  • Ongoing data collection for timely and user-friendly feedback to support learning and adaptation, including critical analysis on gaps in implementation and risk assessment of potential service delivery or trial failure points.
  • Working in partnership with the Neighbourhood Centres in alignment with the “Our Shared Vision” strategic framework, which outlines the programmatic intention to be place-based in service delivery and operational model.
  • Co-hosting of a quarterly Community of Practice for the Link Workers, in partnership with the department.
  • Working in close collaboration with the Queensland Social Prescribing Expert Advisory Group.
  • Regular project meetings with the department throughout the life of the trial.
