The key objective of the service is to establish a 10 bed MHACPI transition unit within a residential aged care facility in Northern Sydney Local Health District. The target group for the transition unit is people with severe behavioral and /or psychiatric symptoms associated with mental illness and / or dementia; and complex needs due to mental illness and or/dementia and ageing.
