Description

The Supported Accommodation Service for Homeless Adults focuses on addressing the accommodation support needs of people who have a serious and persistent mental illness (see appendix 1) and who are homeless. The aim is to provide these adults (18-65) with individualised psychosocial and clinical support within a transitional accommodation facility for a period of up to 12 months.
The facility is situated on Moore Street, Perth and its location reflects the needs of residents and potential residents, with good access to mental health services, public transport and other community services. It is within close proximity of drug and alcohol treatment services, and existing agencies that support homeless people.
The facility will have 24 hour 7 days per week on-site non-clinical support provided by a Non-Government Organisation (NGO) that will be appointed by the Department of Health (DoH) through this tender process.
Residents will have access to in-reach clinical services provided by the local mental health service.
The program is not a catchment based service and will accept people who meet the entry criteria from wherever they are located.
