Description
The Disability Services Commission is planning to fund the establishment of a four year Children’s Continence Management Service Pilot Project.
We wish to gauge the level of interest within the not-for-profit community services sector in providing the service. If you are or might be interested in providing the service at some future time, please complete and return the Registration Form attached here by 2:00pm, 15 August 2012 (Perth Western Australia).
You will also find information relating to the potential service requirements on page 3 of the Registration of Interest form.
Please also note that there will be a non-mandatory information session held on 20 August 2012 at 11:00am at the Commission, 146 Colin Street, West Perth.
