Development of an Outcomes Mapping Product & Outcomes Library The purpose of this project is to design and deliver • a tangible methodology and solution that maps department-wide outcomes at service, program and portfolio level for individuals, community and systems, • an outcomes library that provides the basis on which outcomes can be applied to current contracts and selected and applied to future commissioning activities, • a change and implementation plan to support Communities to implement across current contracts in place, and • a knowledge transfer package to enable handover from the project into business as usual for ongoing delivery
