Overview and Scope
The Friend In Need – Emergency (FINE) scheme is a 2008 election commitment of the Liberal Government in Western Australia to work in partnership with health and community care to help older and chronically-ill patients to give them an alternative to hospitalisation.
The FINE scheme is comprised of three interlinked components:
• community based non-inpatient acute and complex care
• a strengthened network of care coordination (case management)
• community based flexible care packages
The North and South Metropolitan Area Health Services will be providing the supporting component of “a network of care coordination”.
The DOH will assume responsibility to ensure that General Practitioner engagement is maximised in all aspects of the FINE scheme.
This EOI is specifically for the provision of services supporting the FINE scheme’s “community based non-inpatient acute and complex care” component for the period 1 July 2010 to 30 June 2012. The three services that are to be provided by the successful proponent will include:
• Community managed Hospital in the Home
• Post Acute Care
• Community Nursing
These services are to be provided consistently across the Perth Metropolitan area.
Appropriate clinical and medical governance will be the responsibility of the service provider of these services. It is expected that all aspects of the services will incorporate a multidisciplinary approach and that the needs of carers be incorporated into the profile of care for each of the three services.
