Description
The Program aims to address the gap in the integration of services and transition between primary care and cancer services, thereby reducing the mortality and morbidity rates for Aboriginal people. The project streams will be carried out over a 4-year period across all of NSW. The Program will be delivered in two concurrent project streams described below. As outlined earlier, this RFQ is for an evaluation framework for the Program (covering both projects 1 and 2) as well as for the completion of the mid point and final evaluations.
