Description
WA Country Health Service (WACHS) requires the provision of specialist palliative care services for the Pilbara region.
WACHS seeks to engage a Contractor to provide the services of a specialised, experienced, and established multidisciplinary in-reach Palliative Care Team to deliver an innovative service model and comprehensive program of assistance and capacity building to the existing WACHS Palliative Care Team in the Pilbara. The Contractor's Specialist Palliative Care Services are to be provided by health professionals who have recognised qualifications or accredited training in palliative care, along with relevant palliative care experience.
The overall objective of the Contract is to improve the quality and sustainability of the Palliative Care Services delivered by WACHS in the Pilbara.
The Contractor specialist palliative care team will comprise the following Specified Personnel:
- Palliative Care Specialist Consultant(s);
- Palliative Care Social Worker(s); and
- Palliative Care Counsellor(s).
