Description
The Disability Service Commission (State Party) is seeking responses to expand the Panel Contract of Disability Sector Organisations (Service Providers) to deliver a range of diverse, innovative and flexible services to people with disability living in the WA NDIS NDIA and WA NDIS My Way trial sites.

Service Providers who have Home and Community Care (HACC) Program service agreements with the Department of Health must apply and be successful in this request process to provide services in the WA NDIS My Way and WA NDIS NDIA trial sites and/or expand their services to include disability services.
To be eligible to apply a Respondent must meet all of the following criteria:
a) Have a current service agreement with the Department of Health (DOH) to deliver HACC Program services
b) Currently deliver HACC services to people living in at least one of the WA NDIS trial site areas as defined below:
Service Providers who are already on the Panel Contract for Individually Funded Services do not need to reapply during this process.
Under NDIS Guidelines, registration to become a NDIS Service Provider must have been through the Service Provider approval process of the relevant state jurisdiction as well as registering with NDIA. Therefore, to be a Service Provider in the WA NDIS My Way and NDIA trial sites in Western Australia a Service Provider must be on the State Party’s Panel Contract for Individually Funded Services.
