Provision of Domiciliary Palliative Care Services

Mackay Hospital and Health Service is seeking Offers from suitably experienced Offerors who possess the capabilities and experice to rpovide Domiciliary Palliative Care Services to clients in the home environment.

The scope of the Agreement includes the provision of the Services to clients, including:

1. provision of case management;
2. provision of nursing services including administration of medications, complex symptom management and after-hours care where required;
3. provision of personal care services;
4. liaison and partnership with MHHS, including managing transitions into and out of hospital where appropriate;
