Transition Care Services for the Transition Care Program

The Department of Health is seeking to establish new contractual arrangements for the provision of the TCP services and capacity that will meet future TCP demands noting the ageing population and demand pressures of patients in public hospitals awaiting access to TCP.

This procurement will establish:

• A Panel Arrangement with providers that demonstrate capability and experience in delivering aged care services and/or TCP services, and demonstrate current and/or potential future capacity to provide TCP and similar hospital patient transition services; and
• An initial tranche of TCP Service Allocations for both Community and Residential places. In relation to TCP Residential, at specific facilities (known as “Residential Places” under the Transition Care Program Agreement) nominated by Respondents.
