STATEMENT OF REQUIREMENTS
A bio-psychosocial model of mobile community case management will be used to provide intensive support to HIV/AIDS clients who experience chaotic and transient lives. This will include working with medical teams to encourage compliance with HIV medication, mental health, sexual health and any other health matters including problems relating to sexuality and safe sex practice. Professional support will also be offered when necessary in areas such as housing, problematic substance use, finances, legal and relationship based issues. The service provider will work with 24-26 clients with particular emphasis given to working with Aboriginal and culturally and linguistically diverse (CALD) clients.
