Description
The Disability Services Commission is seeking to contract a not-for-profit service provider to develop and implement the Neurodegenerative Conditions Coordinating Care Program (NCCCP). This program will provide timely and appropriate support to people with rapidly degenerating neurological conditions across Western Australia to continue to live in their own home for as long as possible. These supports are to be complementary to the range of services already available in the community to support the person to live at home. It is not intended that this program will meet all the needs of the target group.
