Description
1.1 The Transition Care Program, Mackay Hospital and Health Service (MHHS), seeks offers from established and experienced Non-Government Organisations (NGO)’s) to deliver a range of services within the prescribed Community locations.
1.2 Services are required to be delivered in the locations serviced by the Mackay Hospital and Health Service Transition Care Program (TCP) being Mackay, Sarina, Dysart, Clermont, Moranbah, Whitsundays, Bowen and Collinsville.
