Description
To improve vaccination coverage rates in residential aged care homes (RACHs) including residential aged care facilities, enhancing catch-up vaccination activities, addressing barriers to vaccination, and optimising equitable access. The project’s objectives include: 1. Supporting RACHs to verify and monitor residents’ vaccination status by • designing and providing a vaccination tracker that utilises the AIR database and exchange reports to confirm residents’ vaccination histories • enabling and supporting the tracking of individual resident vaccinations, upcoming due dates, and consent status through the vaccination tracker spreadsheet and Power BI dashboard. 2. Standardising consent processes by: • introducing and promoting a standardised online consent form for RACHs • advocating for adoption of the consent form through stakeholder engagement with GPs, pharmacists, and aged care providers. 3. Enhancing stakeholder engagement and education by • strengthening communication with GPs and pharmacies visiting aged care sites to introduce tracker usage and advocate for standardised consent form covering all recommended vaccinations for residents • delivering education sessions to aged care facilities on using the tracker and consent requirements. 4. Developing and sharing resources by • creating and launching an online RACHs Toolkit featuring links, learning modules, videos, and resources to support immunisation activities • promoting resources and project updates using the online platform. 5. Supporting implementation and continuous improvement by • conducting site visits to assist with AIR applications, tracker installation, and troubleshooting. • monitoring tracker utilisation and vaccination clinic activity, providing ongoing support to ensure effectiveness and sustainability
