Provision of Skin Cancer Screening Services

The Department of Agriculture and Food (DAFWA) is seeking Contractor(s) to provide skin cancer screening services to DAFWA employees. Contractors are requested to submit an Offer for either Category A or Category B services.

Category A - DAFWA Facilities in Perth metropolitan and all Regional Locations
Category B - Selected Regional Location(s) Only
DAFWA intends to appoint and maintain only one Contractor to Category A. This Contractor will be required to provide the services to DAFWA staff at DAFWA’s Perth metropolitan facilities. The Contractor is also to have capacity to provide the services at all of the DAFWA regional facility locations listed in Schedule 2- Specification/Statement of Requirements. DAFWA will later advise this Contractor on an annual basis, of the DAFWA regional facility locations where their services will not be required.
DAFWA envisages appointing other Contractors throughout the term of the Panel Arrangement under Category B, that can provide the services at one or more of DAFWA’s regional facility locations listed in Schedule 2- Specification/Statement of Requirements.
