Description
The proposed ‘Statewide expansion of Diabetes Connect’ grant agreement will enable the regionally piloted Diabetes Connect service to be scaled up to include metropolitan General Practices, establishing a statewide, all ages access to the service. Expanding the service is anticipated to help people living with diabetes in Western Australia (WA) better manage their condition in the community with the support of General Practitioners.
