Description
The Department of Human Services (DHS) is seeking to engage an organisation to provide direct transportation services to frail, older people aged over 65 years or over 50 years for Aboriginal and Torres Strait Islander (ATSI) and transport, information, referral and brokerage services to younger people aged under 65 years or under 50 years for ATSI with disability and their carers with transport services that supports their access to the community.
The Murray Mallee Community Passenger Network (CPN) delivers services in the area of Coorong District Council, District Council of Karoonda East Murray and Southern Mallee District Council.
The Murray Mallee CPN is required to:
Deliver 1,552 transport services (one way trips) annually, which are those where the trip is provided by a worker or a volunteer
Indirect transport services including trips provided through vouchers
Use existing commercial transport services and resources in the region to meet identified transport demand where appropriate and affordable
Recruit, train and support volunteers in accordance with the Department of Planning, Transport, and Infrastructure (DPTI) Volunteer Driver Accreditation Program (VDAP) and standards set by the DHS.
Provide the necessary accommodation, administrative and accounting support to the CPN Regional Co-coordinator
Ensure the use of the Customer Relationship Management system (Salesforce) as the co-ordination and booking management tool and the data depository for CPN processes where applicable
Ensure the use of the Commonwealth’s My Aged Care portal as the method to receive referrals for direct and indirect transport for CHSP clients aged 65 and over and/or ATSI people aged 50 or over
Review annually and prepare an annual report on project outcomes achieved during each financial year
