Provision of Transition Care Services for the Transition Care Program (TCP)

Description
Transition Care provides short-term support and active management of older people with chronic and/or complex care needs at the interface between the hospital and home or with the aged care sectors. Older people can require more time and support when moving from hospital to home or aged care to aid their best recovery and help them remain independent. As at November 2016, there are already 346 TCP places contracted in WA; 296 in the Perth metropolitan area and 50 in regional areas.
Through this Request, the Department is seeking to engage suitably accredited providers to establish a panel for the provision of Transition Care Services for the Transition Care Program (TCP) within the Perth Metropolitan Area. The required services are delivered within a Residential setting, and within the following Metropolitan Health Service regions:
• East Metropolitan Health Service (EMHS);
• North Metropolitan Health Service (NMHS); and
• South Metropolitan Health Service (SMHS).
Respondents may submit an Offer with respect to one (1) or more of the above three (3) specified Health Service regions.
The Head Agreement established as a result of this Request will constitute a standing offer, whereby the Department, subject to funding and approval, will be able to seek quotes on an ‘as required’ basis during the Term; enabling effective future demand management.
It is anticipated that the immediate award of one (1) or more customer contracts for up to approximately seventy-five (75) places will occur resulting from this procurement to meet current urgent demands; with a preference for places located within the East Metropolitan Health Service region.
DoH2016552 is expected to commence on 1 May 2017.
