This Request is for the supply and delivery of Vascular, Cardiac & Renal Access Grafts and Patches to Western Australian Public Health Care Units.
The Head Agreement established as a result of this Request may be awarded in any of the following ways:
• a single supplier for all items in a range;
• a single supplier for individual items; or
• a panel of suppliers.
The current supply of these devices has been on a consignment basis with company representatives regularly monitoring and replenishing devices as they are used. It is an important requirement of this Contract that this type of arrangement continues.
