Description
The Department of Health on behalf of WA metropolitan Health Service Providers is seeking to establish a Standing Offer Arrangement for the provision of Interim Hospital Package (IHP) home support services. Under this arrangement, Customers will place orders on an ‘as required’ basis.
IHP services are purchased by individual hospital sites, with local clinical/operational decision-making including how many services to purchase.
IHPs enable public hospitals to purchase a range of in-home support services for patients over 18 years of age who would otherwise remain in hospital or require readmission to hospital, often where there is an absence or delay in accessing usual community services. Services can be up to 12 weeks and typically include supporting a patient with personal care, domestic assistance, and/or a ‘discharge to home’ settling type service.
The Head Agreement established as a result of this Request will be awarded as a panel of suppliers.
