Description
The proposed ‘Comprehensive type 1 diabetes self-management, education and support program’ grant agreement will enable the delivery of key services that enable people with type 1 diabetes to self-manage their condition and prevent hospitalisations. It is anticipated that the program will help shift Western Australian (WA) type 1 diabetes care from reactive, acute-care interventions to a proactive, community-based model that empowers people with type 1 diabetes by providing comprehensive education, psychological services and peer support networks.
