Description
The Diabetes Pilot Program is funded under the Better Community Health initiative of the Fourth Community Pharmacy Agreement between the Department and the Pharmacy Guild of Australia.The Diabetes Pilot Program will be implemented in two overlapping stages and will rely on the evaluation of Stage 1 activity to inform the implementation of Stage 2.
The evaluator will be required to develop an evaluation framework for Stages 1 and 2 of the Pilot Program.This includes the development of pharmacy data collection requirements and reporting templates to be delivered during Stage 1 of the Pilot Program.
