Description
This Request is for the Supply of Denture Teeth (on consignment) to Dental Health Services (DHS) and is established as a replacement contract for HCNS230811. Consignment will be to 13 adult dental clinics located in both metropolitan and country WA. These sites include, Albany, Armadale, Bunbury, Busselton, Cockburn, Fremantle, Joondalup, Morley, Mt Henry, Rockingham, Swan, Victoria Park (Liddell) and Warwick.
The scope of the contract will include the following categories:
Teeth – Acrylic Resin Multilayer (Upper Anterior)
Teeth – Acrylic Resin Multilayer (Lower Anterior)
Teeth – Acrylic Resin Multilayer (Upper Posterior)
Teeth – Acrylic Resin Multilayer (Lower Posterior)
Replacement Shade Guides
Individual Shade Tabs
It is anticipated that award will be to a single Supplier. The resultant contract will be mandatory for DHS state-wide.
