Description
The Department of Health (the Department) is seeking to establish a Panel Arrangement for the provision of independent quality reviews of transition services.
There are two categories of transition services within the scope of this arrangement; those delivered under the Transition Care Program (TCP); and those delivered under the ‘From Hospital to Home’ (FH2H) disability transition support service. Suitably qualified assessors are required for each type of transition service.
