Description
The Department of Health is seeking a Tenderer to develop a suite of educational resources for general practices and Aboriginal Medical Services to assist practices to adopt the Health Care Home model.
The successful Tenderer will be required to provide a package of training resources, comprising 11 modules (including an overarching Handbook and 10 self-paced learning modules) that are based on Bodenheimer’s 10 Building Blocks for High-performing Primary Care. In addition, this organisation will also develop complementary resources for each module, a training curriculum, and a patient resource.
In addition, the successful Tenderer will also be required to provide training and support to another organisation to deliver the training to the Health Care Homes (i.e. a train the trainer arrangement).
