Description

The WA Department of Health requires the provision of voluntary community-based case management support services for people living with HIV who have chaotic and complex lifestyles.

The successful Service Provider will be expected to provide intensive case management support services to the clients as part of their ongoing HIV management. Services and support should be provided in a manner which contributes to reducing the stigma and discrimination associated with HIV.

The intended community outcomes are aligned with the state and national HIV strategies and aim to:

  • Reduce the transmission of, and morbidity and mortality caused by HIV
  • Minimise the personal and social impact of HIV.

Service Level Outcome(s)

The Service Level Outcomes (SLOs) described below are the outcomes that a service provider will work towards achieving over the duration of the contract. These were developed in conjunction with relevant stakeholders and are intended to guide the service model that is proposed by Respondents. The State Party will also take into consideration any additional SLOs that may be proposed.

SLO1: PLHIV who have chaotic and complex lifestyles have voluntary access to and are actively engaged with HIV clinical services, improving their health outcomes and quality of life.

SLO2: PLHIV who have chaotic and complex lifestyles with high support needs are supported to access a range of psychosocial welfare and support services tailored to their needs to minimise the personal and social impact of HIV.

SLO3: All clients are offered a set of comprehensive psychosocial interventions (e.g. individual and group counselling and education, peer support, family counselling) to support behaviour change, for example, safer sex practices, bringing them to a point of readiness for treatment initiation or improving medication adherence.

SLO4: A risk management strategy is established and defined processes are followed for non-adherent clients to reduce the risk of onward transmission of HIV.

SLO5: A risk management strategy for staff is established and defined processes are followed to manage potential risks to staff.

SLO6: PLHIV with chaotic and complex lifestyles are supported by suitably qualified and appropriately trained/experienced staff.
