Description

WA Health is seeking Service Provider/s to coordinate the Healthy Lifestyle Program (HLP), to support people at risk of, or newly diagnosed with, a chronic disease to improve the management of their health and wellbeing.

Service Providers will have the knowledge and capacity to coordinate an approach that is responsive to the needs of the local target populations within the local area.

There will be two Service Agreements to deliver the HLP across the Perth metropolitan area:

  • North Metropolitan Area Service Agreement
  • South Metropolitan Area Service Agreement

Refer to Attachment 1 Service Agreement Area Boundaries.

Services in regional and rural areas are supported by the WA Country Health Service (WACHS) and are outside this Service Agreement.

WA Health will engage an external researcher to support the collection, analysis and production of relevant patient and service data in order to maintain the longitudinal database and research evaluation for WA Health.

Service Delivery is currently met through the Better Health Improvement Program (BeHIP) Service Agreement with Medicare Local with current arrangements to cease on 30 September 2015.
