Description
The South Metropolitan Heath Service (SMHS) is developing a comprehensive Heart Failure Management Service. SMHS envisage this will be a multidisciplinary service working across hospital and community sites (across the continuum of care) within the South Metropolitan Area.
This Tender has been developed in order to canvass the range of interested parties relating to the provision of a community based care coordination Heart Failure service within the SMHS area.
