Case Management and Support Coordination - Disability Services Continuity of Support

The Queensland Government through the Department of Communities, Disability Services and Seniors is seeking Proposals for Case Management and Support Coordination services for Disability Services Continuity of Support.

The Queensland Government is seeking to engage a single Supplier (or cooperative network with a lead provider) to deliver a case management and support coordination service to Continuity of Support (CoS) clients; and to work collaboratively with these clients, their families/guardians and their service providers to ensure that the supports being provided are, and continue to be, appropriate, stable and responsive to the person’s needs.
