Communities is seeking to engage with service providers to deliver the Housing First Support Services under the Housing First Homelessness Initiative (HFHI). This is the second integral part of the delivery of the HFHI across the Perth Metropolitan area, Rockingham, Mandurah, Bunbury and Geraldton. This request is for the provision of the Housing First Support Service that will work in concert with the Housing First Support System Coordinator under the Housing First Homelessness Initiative, forming a collective impact approach to better support people who are sleeping rough. The Housing First Support Service is not a standalone service and the success of the overall initiative is dependent on all the other parts of the initiative working together. Services required under this request include assertive outreach, case management and after-hours support.
