Community Support Service and Community Connect Trial - Manoora-Mooroobool

The purpose of this procurement activity is to purchase two services to operate in the Manoora (306021151) and Kanimbla-Mooroobool A2 (306021149) SA2 catchments of the Far North Queensland region.
This will be achieved under the Community Investment Specifications under the, Access – Community Support (T101) and Access – Information, Advice and Referral (T103).
• Service type 1 – an Access - Community Support service (Neighbourhood Centre Program)
o Access – Community Support (T101), delivered under the Community Investment Specifications V3, provides for service delivery which enhances community capacity for individuals and groups, and promotes greater public awareness of social issues for people who live in a defined geographic area. The aim is to respond to identified and emergent needs of the local community, with a priority on vulnerable individuals and families.
o This service will be delivered in the Far North region for the period 1 September 2017 to 30 June 2019 for the contracted price.
• Service type 2 - Community Connect Trial
o The Queensland Government is trialling the placement of Community Connect workers in high need communities to facilitate support and referrals for individuals and families to specialist services such as those that support people with complex needs and those affected by domestic and family violence. The Community Connect trial aims to promote and encourage resilience in vulnerable individuals, families and communities.
o This service will be delivered under the Access – Information, Advice and Referral (T103) service type under the Community Investment Specifications V3, which provides for service delivery that assists service users and community members to make informed decisions about, or be connected to, the services and support they need and the additional requirements relating to the Community Connect trial (see attachment 1).
o This service will be delivered in the Far North region for the period 1 September 2017 to 30 June 2019 for the contracted price.
The specified service is to be centre-based and located in the Manoora SA2 (306021151) and Kanimbla-Mooroobool SA2 (306021149) SA2 catchments.
The successful Supplier is expected to operate from the existing neighbourhood centre located at Manoora (29 Marchant St, Manoora), with outreach to the Mooroobool Centre (2 Long St, Mooroobool).
