Particulars
This contract is for the provision of Specialist Aftercare Program in Murrumbidgee to improve life outcomes for care leavers who, as a group, have poorer outcomes than their peers. Providers work with care leavers to address their needs, connect them to services and supports and develop foundational skills that build resilience, connection, confidence, and capacity in areas which will assist in successful transition into adulthood. Providers review and update aftercare plans to ensure young people have financial support and entitlements and any new referrals or connections they may need.
