Head to Health Kids Hub

The Mental Health Commission (Commission), as the State Party, is working with the WA Primary Health Alliance (WAPHA) to develop and implement a WA H2H Kids Hub, a key priority action from the Federal Government's 'National Children's Mental Health and Wellbeing Strategy'. H2H Kids Hubs are intended to operate as a child mental health and wellbeing service, targeting 0-12 year old infants and children with mild to moderate emerging complexity.

H2H Kids Hubs aim to:

• provide comprehensive, multidisciplinary care which supports children and their families;
• improve early intervention outcomes for children’s mental health and wellbeing; and
• complement and enhance existing services provided to children and their families.

The Commission is establishing the H2H Kids Hub in the City of Swan to complement and integrate with existing child health and family services already provided in the catchment. A WA Service Model has been developed, based on the Head to Health Kids - National Service Model. The H2H Kids Hub should be delivered in alignment with this localised model of service (attached).

The Commission is seeking to appoint a single Service Provider to deliver the H2H Kids Hub service, tailored to the needs of the community and families within the City of Swan.
